Hospital discharge arrangements for older people in Wales
We have a detailed factsheet on hospital discharge arrangements which explains what you should expect from staff involved in planning and arranging your discharge from hospital. This includes the processes which should be followed to ensure that people receive appropriate further assistance once they have left hospital – be this on a temporary or ongoing basis. This might include:
- other NHS services; and/or
- help from the local authority social services department; and/or
- services from other organisations (for example, the charity or voluntary sector).
